Its followers speak an incomprehensible language, wear creepy cassocks and worship Chernobog. The hero is an ordinary guy named Caleb, who is told about the cultists by his girlfriend Ophelia. The girl herself joined the ranks of the Cabalists, so the boy has no choice but to enroll there himself. At first, nothing alerted the protagonist, and he even liked it, because his beloved, along with a couple of people, were made the Chosen Ones, who were honored to meet Chernobog. The deity suddenly accuses the chosen ones of betrayal and demands immediate punishment. Caleb was buried in a grave, from which the murdered man soon rose - resurrected, wanting revenge. Taking a pitchfork, the character goes to inflict cruel, bloody retribution on everyone. During his absence, the Cabal itself has spread throughout the country, but a guy with a shotgun, a sawed-off shotgun, a Tommy gun, a pitchfork, a flare pistol, a lighter can, a magic staff with a skull, a voodoo doll, and a huge desire can easily deal with everyone, giggling evilly after each murder. Blood consists of levels that are divided into episodes, presented as long corridors with a lot of secrets and doors behind which they are stored, as well as insidious traps and no less dodgy opponents. To move between segments of the level, the player searches for keys, and at the end - activates the lever to move to the next level. In the process, the hero conducts shootouts with opponents - different types of spirits that can become intangible and resistant to bullets, zombies that can be resurrected again, spiders that spray poison, and cultists.
