Cold Fear tells the story of a member of the Coast Guard named Tom Hansen, who went to investigate a mysterious Russian vessel. The plot tells that the ship's crew gave strange distress signals, without specifying what exactly happened. After the protagonist and his comrades get there, all members of the group, except Tom, are killed. The user must stay alive, simultaneously destroying enemies encountered along the way and solving various puzzles. It is proposed to understand what happened, as well as encounter dangerous monsters, and then try to find their lair. During the passage, it will be possible to visit many locations inhabited by opponents.Cold Fear features a variety of secondary characters that the player is invited to interact with as they complete tasks.
