Only A Dream takes players back to 1990. The story is about a guy named John Simmons. The hero tried to start a new life after his parents' divorce. His mother left the family 3 years ago for a lover and cut all ties, and his father fell into a deep depression because of it and disappeared a month ago. The protagonist repairs an apartment he inherited from relatives and finds a steady job. But he does not enjoy his success for long. One day, the main character opens his eyes at three in the morning and feels strange. Then he tries to sleep again. Afterwards, the man wakes up in a place that is unfamiliar to him - a network of caves and underground corridors. Now it's up to him to find a way to get out of these mines.

Only a Dream is a hybrid of a horror, walking simulator, and first-person action game. Gameplay consists of exploring locations, finding a way out, collecting and reading notes, riding on wagons, overcoming obstacles, fleeing from the pursuer. The backstory is revealed through notes scattered around the map left by the protagonist's father named Frank. In the game you can walk, run, squat, interact with certain objects, use a flashlight with a limited charge. Running consumes little stamina. The video game also features heart-pounding mechanics. At high heart rate (when the mark reaches 120) you can get a heart attack and lose. Game progress is saved only in the camp.
