The events of Painkiller: Recurring Evil continue the story of the previous part of Resurrection. The user takes control over Bill Sherman, who becomes the main thing in purgatory. The protagonist has a powerful force due to the Sword of Seraphim. The fallen angel Samael rules Hell and steals the protagonist's weapons, throwing Sherman into unknown locations of Purgatory, again generating battles. The ending of this part may change. Bill will be defeated by Samael, he will be saved by Belial, or Sherman will kill the fallen angel.The gameplay remains unchanged, the user gets to locations strewn with demons with whom he will have to fight. As he progresses, Bill's inventory increases. He acquires new guns and uses a shotgun, a hammer, a bazooka, machine guns and other types of weapons. Each gun has a secondary firing mode. No new monsters, all opponents were previously in the previous part. The gamer will need to go through 5 levels to get to the ending.In total, more than 5,000 opponents will have to be defeated. Many useful items are scattered in each area, and enemies climb from all sides. They have their strengths and weaknesses. This part has improved artificial intelligence, elaborate environment and graphics. The gameplay will last about 5 hours. Each level is long and often changes design and terrain.
