Emphasis is placed on first-person shooter mechanics with horror elements, plunging into the terrifying and dark world of an alternate universe mired in experiments on living beings and humans. Players take the role of the protagonist, who wakes up without memory on an unknown island and tries to escape by finding a way to freedom.

The plot of Resident Evil Survivor tells the story of the protagonist Thompson, who gets in a plane crash and survives imprisonment in a cursed land. The land around is washed by the waters of the ocean, making it difficult to escape and you need to find a way to swim or fly away from here as soon as possible. An area called Shin has been infected with a deadly T-virus, subjecting the local population and animals in the area to terrifying mutations. The protagonist, who has completely lost his memory due to amnesia, tries to figure out what's going on in the locations by exploring and progressing through the story.

The player has to make his way through terrifying locations, forcing the atmosphere and to observe the utmost care not to be caught by monsters. The character has access to inventory and the ability to pick up weapons scattered around sections of corridor maps. When hiding from your opponents is not possible, the moment of battle comes, forcing you to waste precious ammunition on your opponent. Shooting back must be done wisely, otherwise there is a chance to be left unarmed and count only on escape.
